NHS care plan letter explained

A care plan letter sets out the agreed approach to managing your health condition — what treatment you'll receive, who is responsible for your care, and what the goals are. You have the right to be involved in it.

What a care plan covers

  • Your diagnosis or condition — what is being managed
  • Treatment plan — what care you will receive and how often
  • Who is responsible — your named care coordinator or lead clinician
  • Goals — what the clinical team is aiming to achieve
  • Review date — when the plan will be assessed and updated

Your right to be involved

You have the right to be involved in developing your care plan. The NHS Constitution states that patients have the right to be involved in decisions about their care. If a care plan has been produced without your meaningful involvement, you can ask for a review meeting.

You also have the right to request a copy of your care plan at any time and to request changes if it does not reflect your wishes.

If you disagree with your care plan

  • Write down specifically what you don't understand or don't agree with before any meeting
  • Ask your named care coordinator to explain any section you are unsure about
  • If your concerns are not being addressed, contact PALS at the relevant Trust
